Co-payments
We request copayments at the time of service. For any service not covered by your insurance policy, you will be responsible for the cost of treatment.

School Forms And Referrals
WPA does not accept faxed school/camp/health forms, please mail or drop off forms directly to the office. Forms are completed every Thursday unless they are brought with you to the physical appointment. Please give us at least one week notice for referrals.

Fees
Missed appointments: $50 (Kindly give 24 hours notice if unable to keep your appointment.)
Returned checks: $35
*Copying medical records: $25 (Please allow 4-6 weeks to complete this request.)
*Completion of basic school/health forms: $10 (More complex forms may incur additional fees. Please allow 1 week for completion of all forms.)

*If expedited requests are needed, please send an additional $10 rush fee with your original request.
